FEES:

• To allow me to deliver care in a timely, thoughtful, and thorough fashion my practice is based on fee-for-service. No insurance payments are accepted, but I am happy to provide you with the forms and information you need to bill your insurance company, if you so desire.
• Payments due are as follows:
  • Initial Evaluation: $290.00
  • Follow-up visits: $90.00
• Payments are due at the time of service and may be made by cash, check, or credit card.
